Today we’d like to introduce you to SARAH MIRKIN.

Hi SARAH, thanks for joining us today. We’d love for you to start by introducing yourself.
I’m passionate about helping people improve their health and transform their lives. As a registered dietitian, I specialize in resolving digestive issues such as irritable bowel syndrome (IBS) and small intestinal bacterial overgrowth (SIBO), and in guiding people to lose weight in a sustainable, lasting way. My mission is to make sure no one feels stuck, dismissed, or without answers on their health journey.

My passion stems from personal experience. In high school, I fell into the trap of restrictive dieting, pushing myself to extremes with exercise, and staying on a strict fat-free diet for five years. When I eventually let go of those rigid rules, began eating normally, and exercised in moderation, I lost weight naturally. It was amazing to discover there was no magic bullet. The answer to achieving a healthy weight was simply listening to my body, nourishing it, and fueling it properly.

Unfortunately, those years of restriction took a toll on my gut health. I developed IBS and, like so many others, felt confused and frustrated while searching for relief.

When I was working as a dietitian at Cedars-Sinai Medical Group in the early 2000s, I read about Dr. Mark Pimentel and his research showing that most IBS cases were actually SIBO. I became his patient and was one of the first people to take the hydrogen breath test for SIBO. At the time, hydrogen was the only gas we knew to test for. Since then, he has developed a much more accurate test that measures three gases.

I tested positive for hydrogen. The only treatment we knew of in 2004 was an elemental liquid diet, which Dr. Pimentel believed would starve the bacteria to resolve SIBO. So that’s what I did. I felt amazing during the treatment and immediately afterward, but a few months later, it came back. Treatment alone wasn’t effective for me without dietary changes. At the time, the low FODMAP diet hadn’t been discovered yet. When researchers developed it by 2010, I was thrilled to finally have some answers.

FODMAPs are fermentable carbohydrates that bacteria feed on. Without learning this diet, it’s very difficult to pinpoint which foods might be causing symptoms. When someone with IBS eats a high-FODMAP food, they may not have symptoms for many hours or even days. I was slowly able to restructure my diet as foods were tested for FODMAP content at Monash University in Australia. Ultimately, it was the combination of medical treatment and diet that finally made the difference.

My life became so much better. I was able to live carefree. I realized how many other people were probably suffering in silence without answers, and I knew I could help them too.

Today, I bring that combination of advanced clinical knowledge and real-life empathy to my practice. I work closely with my clients to get to the root of their symptoms and create personalized, practical plans that restore balance and comfort. Whether it’s resolving chronic digestive issues, identifying hidden triggers, or learning to eat in a way that supports permanent weight loss without deprivation, I’m there every step of the way.

I believe in helping people nourish their bodies rather than punish them. My goal is to empower clients to understand what works for them as unique individuals so they can feel confident, healthy, and free from the exhausting cycle of quick fixes and symptom-chasing. Over the years, I’ve seen so many people who thought they’d “tried everything” finally find real relief and sustainable change—and there’s nothing more rewarding than seeing someone reclaim their health and their joy.

During my journey as a dietitian in Los Angeles, I’ve had the opportunity to serve as a media spokesperson, appear on live news segments, work as the on-camera dietitian for a reality show, and help celebrities and even royalty achieve their nutrition goals. I’ve also collaborated with chefs all over the world to help clients make their nutrition plans realistic, delicious, and sustainable—because food should be something you enjoy, not fear.

I’ve learned that good nutrition isn’t about harsh rules or endless restriction. It’s about nourishing your body with healthy foods in a balanced, sustainable way so you can truly thrive. That’s why I wrote my two books: The Beginner’s Guide to Low FODMAP, to help people with IBS and SIBO find practical relief, and Fill Your Plate, Lose the Weight, to show that you can enjoy real food, feel great, and achieve lasting health without dieting extremes.

My mission is to share what I’ve learned so that no one else has to suffer in silence or settle for confusing, ineffective solutions. With the right guidance, balance, and care, you really can heal your gut, regain your energy, and enjoy your life.

Can you talk to us a bit about the challenges and lessons you’ve learned along the way. Looking back would you say it’s been easy or smooth in retrospect?
My path hasn’t always been easy or straightforward. Early on, I had to confront my own unhealthy relationship with food and body image. I genuinely believed that health meant strict rules and constant discipline. For years I lived with rigid food restrictions and extreme exercise routines that left me exhausted and ultimately damaged my gut health. Recognizing that what I thought was “healthy” was actually harming me was a humbling and essential turning point.

When I began experiencing IBS symptoms, it wasn’t just the physical pain and unpredictable bloating—it also triggered anxiety and constant discomfort that affected every part of my life. I spent years feeling confused, and frustrated with the medical system. At the time, there were so few answers, and I often felt dismissed or told to just “live with it.” Even after discovering SIBO, treatment options were still developing, and I had to navigate relapses, restrictive protocols, and trial-and-error approaches before finally finding lasting relief through combining medical treatment and dietary changes.

Working in Los Angeles brought its own learning curve. Nutrition here is a crowded space, full of trends and quick fixes that promise dramatic results. Building a practice grounded in evidence-based guidance meant staying true to my values, even when it wasn’t the flashiest or easiest approach. It took time, consistency, and a lot of trust-building with clients and colleagues.

Serving as a media spokesperson, appearing on live news segments, working on reality TV, and supporting high-profile clients—from celebrities to royalty—has been incredibly rewarding, but also demanding. There’s real pressure to deliver advice that’s both accessible and effective without sacrificing science or integrity. Learning how to communicate complex nutrition concepts in a clear, practical, and compassionate way has been one of my most meaningful professional growth experiences.

All of these experiences have shaped who I am as a dietitian. They taught me empathy, patience, and the importance of truly listening to people’s stories. Most of all, they reinforced my belief that real health isn’t about perfection or punishment—it’s about balance, nourishment, and helping people discover what truly works for them so they can live fully and confidently.

So maybe we end on discussing what matters most to you and why?
What matters most to me is helping as many people as I can achieve food freedom and live a healthy, fulfilling life. I want people to feel confident in their choices, nourished instead of restricted, and free from the stress and anxiety that so often surround eating.

It’s about empowering people to understand what works for them—so they can break free from rigid rules, heal their bodies, and truly enjoy food as part of a vibrant, balanced life. Knowing I can help someone move from feeling stuck and frustrated to feeling healthy, comfortable, and free—that’s what drives me every single day.
